This portal was created so Aventus can provide you the best service that can be offered in the industry. Through this portal, seamless and paperless transactions can be carried out on or before visiting our clinics. However, to be able to enjoy the benefits of this portal, Aventus will need to collect personal information from you subject to the terms and conditions you may agree below. What you can accomplish through this portal?
      You can download all clinic forms that may be required of you, provide Aventus feedback and answer the Health Declaration Form (HDF), if applicable.

What information we collect?
    Registration is required to access this portal. For this purpose, you will be asked to provide personal details such as your complete name, birthdate and age, gender, contact details, and address. The name of your HMO Provider, Employer will also be required, if applicable. Other medical conditions may be asked as well such as chief complaints and treatment.

Purposes of Collection.

1. ) to meet my medical and ancillary needs and/or requirements, other programs and services I availed of;
2. ) for the payment of my bills;
3. ) for research and audit purposes, where necessary;
4. ) to liaise with health professionals and Health Maintenance Organization (HMO);
5. ) for collaboration with other medical health provider, where necessary, and
6. ) for purposes required by law.

In general, the information we collect is important to support your care and is also a record of your relationship with the healthcare staff involved in providing your care, e.g. doctors, nurses, administration and clerical staff. This relationship is based on mutual trust and confidence and we will do everything possible to protect and maintain that trust. It is important that the details you provide are accurate and that you let us know of any changes to them e.g. if you change address. If access to HDF is required depending on the state of the COVID-19 Pandemic and pursuant to existing rules, Aventus will also ask you all necessary information for the purpose of ensuring compliance with all health protocols as a means to safeguard your health and the interests of all patients, and our healthcare staff.

To whom we disclose
    Aventus shall keep your record and personal information in strict confidence. Disclosures will not be made to third parties without your consent, unless permitted by law. Nevertheless, if services were availed of through your HMO provider, you understand that disclosure will be made to your HMO Provider and/or your employer, whenever required under a contract between Aventus and my Employer and/or if pursuant to the Healthcare Plan of my HMO Provider. For instance, if the services I availed of is APE or PEME, I recognize the right of my employer to secure copies of my medical records If you have concerns regarding this disclosure, please contact your HMO Provider or your employer. Further, existing rules allow Aventus to disclose this information if duly requested by the Department of Health or if necessary to comply with the Mandatory Reporting of Notifiable Diseases and Health Events of Public Concern Act of the Philippines. Thus, your health information provided through the HDF may be shared and disclosed to public health authorities if conditions require Aventus to do so.

Storage and retention
    Following the Health Privacy Code issued by the Department of Health, Aventus is obliged to keep your health information for a period of fifteen (15) years. A lesser period of retention may be applied in cases where existing rules prescribe the same such as information related to COVID-19. For example, antigen test results will be kept only for a period of thirty (30) days from the date results have been released.

Yours rights
    As we process your personal data, you have certain rights. These are a right of access, a right of rectification, a right of erasure and a right to restrict processing. You may request a copy of your data at any time. Please make such a request by email to the Data Privacy Officer (DPO) at dpo@aventusmedical.com.ph. Please provide the following information: your name, address, telephone number, email address and details of the information you require. If you believe any of the personal data we hold on you is inaccurate or incomplete, please contact us directly and any necessary corrections to your data will be made without undue delay. If you believe we should erase your data, or if you wish to us to stop storing or using your data, please reach out to the DPO. Note that where you have provided explicit consent for us to use your data you have a right to withdraw this consent at any time, subject to exceptions provided by law such as when we are required to store your health information.

Data Breaches
    Should your personal data that we control be lost, stolen or otherwise breached, where this constitutes a high risk to your rights and freedoms, we will contact you without delay. We will give you the contact details of the person who is dealing with the breach, explain to you the nature of the breach and the steps we are taking to deal with it.

Should You Wish to Complain
    You can contact the Data Protection Officer at dpo@aventusmedical.com.ph. You may also file a complaint with the National Privacy Commission via their website: privacy.gov.ph.

Automated Decision Making and Profiling
    We do not use any system which uses automated decision making or profiling in respect of your personal data.
